For *SCAPE to effectively investigate your concerns comprehensively, whenever possible, the following information should be provided:

Type of concern(s);
Details of parties involved (name of staff, person(s), or company);
Date, period, and venue of reported events;
Evidence substantiating the concerns (if any); and
Any other evidence(s) substantiating the complaint and, where possible, contact details.
